College Application: Teacher Evaluation and/or Letter of Recommendation?

Let's use for example the University of Rochester. It says online that "...a letter of recommendation" has to be submitted. Then in the application forms sections of the website, it says, "Give at least one of your teachers a copy of [the Common Application Teacher Evaluation] form to fill out and submit to us."

So, the question is: Is the Letter of Recommendation in lieu of the Teacher Evaluation, or do they go together (It does say, "Feel free to attach an additional sheet or another reference you may have prepared on behalf of this student" in the Teacher Evaluation form)?

I am applying via the Common Application. Also, the Letter of Recommendation has already been written, so I don't want to pester my teacher with more work.
