Carfax car history report help please?

I was looking on craigslist and there was a nice salvage title lifted truck for sale by a dealer. i managed to run a carfax on it but im not too sure what it means

could anybody interpret the report for me please?

also, what do you think a fair price would be for this truck?

is there any way to be 100% SURE what the problem was?

Also, if i do buy this truck I do not plan on selling it any time soon
