Can native English speakers help me to write salary raise letter?

I personally met with my company GM and ask for raise because my supervisor Mr.Bassam always denied my request. My GM asked me to submit this request in written. Please check and advise if this salary raise request is OK.

Dear Sir

In my interview 1.5 years ago, I was told by Mr. Bassam (my supervisor) that I would receive a salary increase after my three months probation or after having a driving license.
As soon as I got the license and completed the probation, I reminded Mr. Bassam that, I was working on my driving license when I joined the company and you mentioned at the time that I would qualify for a salary increase once I had gotten the license. I requested him to please proceed with the raise as we discussed but my supervisor denied a raise that I really deserved.
Sir, you can call a meeting and ask all the departments about my performance and coordination. I am the one who is always meeting the dead lines. All the time I am updating and coordinating with each dept of our company. This is one of my skills that I am a team player and best in human relations & interpersonal skills. I submitted my everyday report to Mr. Bassam on time. I have a full prove of my reports and customers list which I have dealt till date.

University Degree with 5 years of working experience in leading ISPs and IT sector
Cisco Certified Network Professional (CCNP)
Cisco Certified Network Associate (CCNA)
Certified Wireless Network Administrator (CWNA)
Microsoft Certified Professional (MCP)
Advance Diploma in Software Management
Furthermore I had taken classes of PMP and planning to do certification in Cisco Service provider CCIP. And certified wireless security professional.
