bleeding tendency

hi , my dad is 70 years old man , he has bleeding tendency all the time , he is bleeding from every were like lung. GI. stomach . what are the possible couses for that ? drugs? any other things? why he needs addmission every time in the hospital to recieve coagulants like platelets?
