bike chain is jammed in the frame?

bike chain is jammed in between the frame and the last sprocket and every time i put it back on and change the gears it jams back into the frame again, any help? please
 
Remove the jam by loosening the wheel. On your derailleur is two screws. One marked H one marked L (or sometimes unmarked. If that is the case you have to get down and see which one is making contact when you are in the highest [smallest] gear). You will need to have the bike on a stand or have someone hold the back wheel up for you. Look at the derailleur top pulley. and align the chain and that pulley so it is centered on that gear. Pedal the bike while the back wheel is off the ground and test your adjustment.
