beta fish sick, help!!!!!!!?

I've had this problem before, she would just sit in the bowl and not swim around and keep her fins all clamped up. So I bought her some medicine, after a week she was back to normal. Now she's been doing the same thing for almost two months, I started the medicine again but it seems to have no effect whatsoever. I clean her tank out once a week, she's got nice light, she's in an area where she can see people all day so it's not boring. But mostly she will sit on the bottom of the tank floor, or up at the top and just lay in the top of her plants. I feed her 2 beta bites a day and they are so tiny but I didn't want to over feed her. Could I be underfeeding her? What could be the issue, other than the clamped fins and not swimming around, she looks normal.\
 
You are not underfeeding. Over feeding would be anything over 3 to 4 pellets two times a day.

What is her home situation? What sized tank? Does it have a heater? A filter?
A proper betta home will be between 2.5 and 5 gallons at least. Since betta are tropical (warm water fish), it would HAVE to have a heater. Betta can't live healthily or happily in cold water for long. And it, just like ALL fish, needs a filter, which keeps the water moving and fresher between water changes.

If your betta is in a home smaller than 2.5 gallons, or does not have a heater of filter (most medications don't even have an effect at all without a filter present), then that could be the problem.
