Best approach to UI claim . State - not specific. ?

Employer has lied about some details. In written summary , "Employer states he warned employee about treatment of customers. Employer says that he got statements that employee was intentionally rude to customers." (heresay) I think I have all my ducks in a row for my appeal. They have company policy that requires verbal and documented warnings. They did not follow said policy in firing me. They wrote "substandard performance" and gave an accumulation of documented conversations with no dates and no names ( of complainants ) ... basicly heresay in my opinion. What other details should I pay attention to ? There were no verbals and they state I recieved them. The Seperation Form has written instruction to document " warnings and dates " in Summary area. there were no dates, just "substandard performance" check box was checked. There is a statement by an employee that says that she sells memberships and (he) is driving them away! " I'm not saying anyone should lose their employment, but he has brought this upon himself " ... my defaming comments :-)

Any ideas ?
