Without experimenting, I'd guess the smaller diameter straw would be better. You'll get a faster exhaust for a longer period of time.
Not sure what kind of bottle cap you have in mind. Something metallic with a smooth edge will be best. The larger the diameter the less the rolling friction, all else being equal. Use something small, like a straight pin, for the axles.
