At what point is a doctor's visit warranted?

Okay, so just a brief history - I have been riding horses for quite a few years. On Sunday (it's now 1:34 am on Wednesday) I was riding when my horse took a jump and I lost my balance due to not holding the reins or having my feet in the stirrups and I flew off the side of the horse. I'm not exactly sure how I landed but I have a pretty good idea. I believe that I hit my back/leg/hit on the right side and then rolled and snapped my head back and hit it off the ground (I happened to have just taken off my helmet - stupid I know!!). Well since then I have had a lot of swelling in my lower back/hip/upper leg area and still have some pretty intense tenderness on my head. I know that it is expected to be sore and have some swelling after a fall like that but at what point should I see a doctor? Is there a certain length of time after hitting your head that you should see the doctor if you still have tenderness? How about my leg - should I wait it out a little while or is there a large concern of a possible fracture? What other kind of injury could it be?
