Army stuff up? Importance of complying with superiors.?

Ok, so I have to write an essay on the importance of complying with superiors, in the military. Specifically, aus army.
I've got a paragraph on discipline, respect, loyality, and chucked in a couple of references too the battlefield in there, cuz they love that jazz.

However, I want to chuck in an example, in history, where a national army failed to comply with orders from superiors and stuffed up majorally, any failures you can think of?

What about on the other end of the spectrum?

Any other ideas?
