are all car crashes avoidable if you are a good and attentive driver. can there be one if you do nothing wrong?

What i mean by this is if u stop on every red light, do not speed, look over your shoulder when u change lanes, look in your mirror constantly and othe rthings similar to that. or is there a chance u can get into a car crash that is not your fault.
b. i ask this cause my driving instructor told me this and that he has been driving for 40 years and has never gotton into a crash because he follows this.
