Applying for Job, How to Address Work History Related to Bipolar Disorder?

Okay, so I have a job interview Tuesday at a large retail chain, and I'm not sure what to say about the gaps in my employment history. I have been addicted to heroin for almost two years (I already know not to bring that up in the interview) and when I started therapy for my addiction, my shrink suggested that I had bipolar disorder. So I saw a specialist, and got the diagnosis. Butttt as you can imagine, it's been really hard staying clean and staying mentally healthy -- I was prescribed Lithium but I go off it periodically because of the awful side effects, chronic headaches and vomitting. Because of this my moods and general health are not as they should be. I got fired for lateness at one job (didn't list that one on my application) and at another job, I simply quit because I couldn't handle being around my coworkers.

Basically that's all background on my real question: should I tell the truth about my bipolar disorder and the ensuing problems, or should I make up some lie explaining the gaps in my job history? If so, any suggestions?

By the way, I'm nineteen years old.
