Anyone know the name of this young adult book?

I got a book from the young adult section of my local library, about a boy whose family died in a car crash and only his little brother survived and is in a coma, the man who caused their deaths is trying to kill him too. He has to go to high school in heaven and there are vivid descriptions about how heaven is really built up and under strain because of the high population.
Very random book, but brilliant! Any ideas?
