Anyone ever have to fight your insurance company?

Has anyone here ever had to fight your insurance company to pay for a surgery, specifically disc replacement? Although the FDA approved single level disc replacement, most insurance companies, including mine (Personal Choice) are still not paying for the surgery. I have had three opinions on my back and all three surgeons have recommended surgery (DDD at L4/L5 and L5/S1). Two surgeons said fusion, one said disc replacement. Since two level is still not FDA approved, he does fusion on the lower level and disc replacement on the upper level. He has had patients successfully fight their insurance companies, which he has recommended that I do.

My question is....if you have ever had to do this, how does this process work and do you have any tips to win?
