...identities were mixed up? If I remember correctly, it was two girls, and one lived and one died. They got the two identities backwards because the one that lived was so messed up you couldn't tell it was her.
They finally realized the mistake and it was pretty much devastating probably to the family.
Or any similar story would work.
If I could just get a name or a link to a newspaper article, that'd be great.
