Any clue why my fish died, also aquascaping question.?

i have a 55 gallon tank and i am slowly stocking it. Right now i have a total of 5 black phantom tetras, 3 guppies, 9 ghost shrimp, formerly 1 zebra danio. I woke up today and all my fish looked fine then i noticed my zebra danio was missing. i look for at least 15 minutes and couldnt find him. then all the sudden he comes out from under one of the ornaments swimming in circles and its gills looked mangeled. then he statred to swim upward in a spiral motion. then he just fell to the bottom of the tank and his gills stoped moving. After desposing of the fish i checked all my water parameters and they where perfect for the fish. all the fish are tropical community so i dont think there was a fight. I have 3 fiters running on the tank. One for 60 gallon one for a 40 gallon to help anda 15 gallon filter for my hospital tank to quickly cycle it for a sick fish, so he didnt sufficate. I am just wondering if anyone knows how he might have died because it is a mystery to me.


My second question is that i would like to know a good sight to go and get aquascaping ideas because i tried to aquascape my tank but it just doesnt look right.
 
the fish might of got stuck or crushed under a ornaments and it could of damaged its gills. the fish may of already been ill.
