please give me some guidance as to where to look to make an industry-proper query letter for getting a literary agent. I'm so nervous about the query letter I am procrastinating something fierce!

A query letter is the first piece of writing someone will have of you so it is important to polish it and make it professional.

--- Make sure you submit it to the proper submissions editor and spell their name correctly. State your name and what your manuscript is entitled, the genre it falls under, elements of other genres it may include and word count.
--- You will give a short synopsis of your manuscript as you are trying to keep the letter at the extreme most two pages.

---- You should mention any other books and authors that your work could compare to as well as any marketing plans you may have. Try to include any workshops, relevant contests and conferences you've entered and attended. If the speakers are influential you might consider adding their names though not necessary it shows your involvement in the industry as well. It shows your dedication to the business side and not just the actual writing side.

Make sure to include a current address and phone number and e-mail for contact.
Below are several websites with links to query letters as well as tons of other useful information such as lists of credible agents and agencies and horrible agents too. Some have online workshops and a few are agent blogs where they tell horror stories and their do's and don'ts for the submission process.

Most agent websites go so far as to list what they want in a query letter, how long it will take for a response, and other do's and don'ts.
