Antiogen I Conversion (ACE) Serum testing

Hi,

I'm hoping someone on this board can provide me some insight into understanding ACE serum test results. I had a test done last month and it came back high. Normal is 4-60 and my results were 358. I was retested recently and the results were "above the clinical reportable range for this analyte".

I know the ACE can be an indicator of Sarcoidosis, but it could be any nuraber of other things as well. My Neurologist referred me back to my primary doctor. My primary doctor and 2 of their colleagues said they didn't know what to make of it or even what to recommend because they hadn't seen this before and I don't have some of the other inflammatory markers they would expect. I do plan to see my Pulmonologist because I know lung problems can be related, but I had a lung CT a couple months ago that was clear.

Does anyone have any ideas or insights that I can research? I really don't know if this is something I should be concerned about or not. My doctors did agree my results were very abnormal, but what it means noone knows.
 
Hi my ACE was 550 and I needed a lung biopsy...i have scarring that looks like ground glass that my local hospital overlooked...my pulmonary Dr said unless thy suspect something they ignore it....my granulomas are everywhere...and I just roll with the punches I am no longer with any Dr as their help was no help at all..in 90%of time it burns out acording to the Merck Manual I read on line and just avoid bad colRAB as u can get pnuemonia easily...hope this helps...Dee

I haven't been able to find much information on the internet about readings that high.

I did meet with my Pulmonologist. He said it was highly likely I have Sarcoidosis. He said that my symptoms (or lack of severity of them) right now don't warrant any further testing or treatment. He doesn't even want to do a biopsy or lavage to confirm the Sarcoidosis because of the invasiveness of the testing. He went through the garabit of each of the meRAB to treat Sarcoidosis of the lungs and all the side effects of each. He feels right now the treatment would be worse than the disease. I will go back in March for my yearly lung function testing, more bloodwork and another chest x-ray to monitor for any progression. My last chest x-ray in august came back abnormal, but my CT came back normal. I'm not sure what to make of that.

He did say that the disease lasts about 3 years in most people and that research doesn't show much difference in the end of the 3 years if you were or weren't on treatment which is good to know.

Part of my concern though is that none of my doctors really seem concerned about confirming why my ACE is so high. I know Sarcoidosis is only one of the things that can cause it to be elevated, alibet the most likely. I also know I've had lung issues for a long time. I just came off of a coughing/ asthma flare that I had for 4 months prior to this (hence the CT and x-ray).

It sounRAB like kind of a "hurry up and wait" thing with this. It is frustrating that there really isn't a good answer here. I'm glad your doctor was sharing similar information with you.
