Activating Cricket fees?

What are the fees and how much are they to activate a phone flashed to Cricket?

Are the fees payed up front? That is, the activation and the first month's fee?

Are you required to have a credit card whether you activate it at a store front or through a phone call?

typically when activating a CPE (customer provided equipment) you must pay the first month+taxes and a $15 activation fee. Whether or not you need a credit card is dependant on the store. Corporate stores take all forms of payment but Premier stores (indirect) like mine, and other non corporate ooutlets may only accept cash.
