A good diet and exercise plan for a 5'4" 130 pound 13 year old?

I really would like to lose wieght for the summer!
Im probably 130-145 pounds and I want to lose atleast 3o pounds and have a flat stomach but still keep my breasts?
I need my diet to be planned out like what I should eat and what type of food I should eat at my meals and throught the day.
And also exercises that will help too.
I have no acess to a gym but there is a park 2 blocks away from my house so that can be my workout pplace or even my backyard.
I also want my thighs to be less wide and thick.
PLEASE HELP!
I would really appreciate it.
Im kind of addicted to chips and my laptop so have it be something that can add more to my plan.
