A false medical report exists in my MIB disclosure file. How can I get it removed?

If I apply for term life insurance, this false medical information will appear. I have disputed it with the company that reported it and filed a complaint with the FTC. After three years - It still appears in my file.

I cannot get a reasonably priced quote from any insurer because of this fraudulent information. What can I do?
