a book for rating boys?

this is going to sound weird, but i remember a long time ago my friend and i had this paper that was ripped out of a book. and you could rate boys and it would be like in the looks, car, dancing, smile, etc categories, and i want to get it for her for her birthday (as a joke) but since i don't have the book, i don't know where the sheet of paper is from. does anyone remember having or seeing a book like this? it's really important. thanks
