99 Jeep Gnd Chkee Laredo. Slow to accelerate. Tranny feels like its jumping....?

When I hit the gas it feels like the engine is trying REALLY hard. Once at about 20mph it is fine.

Then when I go to stop, it feels like the tranny is jumping back & forth in a jerking motion. When I am stopped- it does the same thing and the car jerks.

Help!!
