91 Honda Accord,automatic transmission won't shift right?

The trans has been fine and one day it just wouldn't shift out of first gear and started slipping in reverse.The strange thing is sometimes it won't move at all,but when I turn off the engine for a few minutes and start it it back,it moves again but still won;t shift out of first gear and no reverse.The fluid has been changed
 
how about the trans filter? did you put the right fluid in it? is it full or is it to low? check these things out.
