2004 Lincoln LS v8 - 4th Gear and R Shift Oddly at Running Temperature?

I have an odd transmission problem...

2004 Lincoln LS -- 3.9L v8
Engine: Jaguar AJ-8,
Transmission: 5r55s

What happens is the vehicle really pushes the gears hard after the vehicle has been driven for any distance further than 3 miles. Before this, everything shifts seemingly okay -- I can still feel the shifts but they're within the range of "normal".

After being driven for about 5 miles the following occurs:

-4th gear, whether going from 3rd-4th or 4th-3rd, will either bang violently or take 4ish seconds to shift (meanwhile it acts as if the engine is idle during this time - If I accelerate while it's in this neutral state the transmission will bang HARD when it finally hits -- I've learned to step off the gas every time it shifts from 3-4). The other gears seem to still have too much pressure but they're not NEARLY as bad as 4th is. If I'm in 4th and it goes directly to 2nd or 1st, it does NOT have this problem. Only 3-4 and 4-3.

-Reverse has WAY too much pressure once the vehicle is up to temp. When the vehicle is cold reverse acts normally -- if vehicle is at operating temp, the vehicle bangs violently when placed in reverse. If I put my foot HARD on the brake flooring it, then hit reverse, it doesn't bang as it offsets the pressure I think? I don't know but it works okay -- if the temp is high enough this doesn't work though.

Does anyone have any idea what the problem could be?

I've had the transmission flushed at a FLM dealership, it didn't fix the problem. It's not a fluid level issue.
