2004 Dodge Ram 1500 4x4- Driveshaft problems?

When I shift my truck to drive and reverse, it makes a humming sound. I had a guy get under the truck while shifting and you can see the driveshaft turn 1/8 when shifting to drive, and turn 1/8 in the opposite direction when shifting into reverse. What's the cause? ALSO, there's a loud CLUNK when putting the truck into Park. Same problem???
