2003 dodge 3500 diesel dually. problem?

this truck has a 5.9 liter diesel 200k miles, I was driving down the freeway with a trailer and a mid size truck on it. I can hit all the gearss no problem, but when I put it in 5th and get it up to 2,000 RPM everything just fails and it brings me down to an idle. and if i try to step on it to raise the RPM's it just backfires and blows smoke.

has anybody had similar problems? or know what the problem is?
 
i ran in to this on a 02 model it was a bad fuel filter causing it to happen to it,under high loads it would literally stall out,i was just starving it out for fuel,id try changing the filter first and draining the water separator on it then see what happens,that may not be it but its good starting point,good luck.
 
Had the same issue with a blocked air filter if it back fires it can blow apart the air filter then ingesting the air filter causing turbo issue and plugging the intercooler.
Also check for water in the fuel filter and change the filter.
