2002 Ford Escape sometimes won't start.?

I'm working on a friends vehicle either a 2001 or 2002 (mfg 09/01) Ford Escape V6. For about a year now it will occasionally start only if the accelerator is pressed. Lately it will die when accelerator is released. The trick seems to be the rev the engine up pretty high. My understanding is there is a puff of smoke out the tail pipe and then it will run just fine. Only happens after sitting for a while (4 hours minimum). Could that be leaking fuel injector. Fuel filter already replaced. Thanks
