2000 honda accord idles rough and then dies?

So exactly a year ago, I was waiting at a ferry terminal and had my car shut off. I went to turn my car on, and it turned on, idled rough and then died. Took it to a mechanic and they said it was my main relay. I had it replaced and now exactly a year later, I'm having the same problem! I'll start my car and it will idle rough, then die. But it will stay started if I give it gas.

Could this be another main relay problem or my fuel pump or something else? I'm just looking for any suggestions! So please if you could...HELP! Thanks.

P.S. I 've also noticed that when it's idling rough, I hear rattling from under the hood, this is also heard when I turn the car off (when it does start and not die).
And yes, I have done and still do the horrible deed of driving with an empty tank. :(
