2000 Ford F-150 v6 w/5 seed manual having clutch problems?

First I had the clutch replaced at 89,000 mi, though it might have been just the master and slave cylinders, it's been 4 years. Now it has 123,000 mi. Most of the time it is fine and I have no problems. However if I have been on the road long enough for the engine to fully warm up, shifting into 3rd creates a grinding sound. A friend jokingly said it sounded like the truck sneezed. It is almost like the engine is still spinning to fast and the faster I'm going when I shift the more pronounced the problem it. I don't try to gun it much, but third is probably the gear I use the most to accelerate and in city driving. Ford experts tell me what's up
