1999 Ford Taurus Stalls after driving 10 miles or while drive in a higher rpm?

This has been happening for a while but it usually fixed itself. But now after driving for about 10 miles, my car will idle extremely low when I stop (the rpm needle is around 500-600).

Sometimes it will cut off when I come to a stop and I often have to rev the engine while waiting. Sometimes while driving it will fix it self and drive normally, and then sometimes the rpm needle will be too high and it will seem as if i'm in a higher gear..

I'm a college student so I don't have money to throw around at the problem... But I've been googling and it seems this is caused by a vacuum leak... Also I'm about to drive from Illinois to South Carolina soon, you think it'll be ok to drive back?
