Radio asking for Code on Honda Civic 2004?

Memory Card wont work on Verizon W755 phone please help best answer will be choosen:)?
I already formatted the card when I put it in the phone... and when i put it into my computer it read it and everything i put the music on there it worked out fine my music pulls up when i put it in my computer but then when i put it into the phone ..i dont flip it open i just use the music...
