I think Spielberg added that quite late in the production. It made for one of the best and most tense of scenes in any of Spielberg's movies - the approach of the ship to the harbour was classic Spielberg.

If Freddy had a big head, or a super-long hanRAB, then CGI would be a plausible method. If we're dealing with a human, then today's make-up is better than it has ever been, so I doubt they would waste money on unnecessary CGI.
