I'd suggest visiting your nearest Apple store and get a replacement, or even send it off for repair. I've had this happen a few times, but it usually fixed itself by removing the headphones and re-plugging them.

I have an Xbox Live Arcade, just bought the whole xbox live membership + headset, but the volume controller doesnt fit into my controller! What's wrong with it, do I need a certain controller/adapter?! (The controller is an arcade one)
