    Question about UGA summer camp health form?

    I'm filling out a uga summer camp health form, and i don't understand some of it. on the second page it says "please complete if you wish UHC to file for reimbursment from your insurance company." what does that mean? and is it necessary to fill out since it says "if you wish"?
