He started a bit earlier than that. When he was questioning the guy after the Commissioner was shot, and Batman stole the coin before he could determine heads or tails to kill him. He was on the way, all he needed was Joker in the hospital to push him over the edge.
In a review I saw a guy get a hooker, then when she got out of his car he stabbed her, then when she was down he kicked and stabbed her again. She was still moving a little bit so he took out a pistol and shot her in the head, no more movement.
