Find another adult - a neighbor, relative, friend - to take the dog to the vet. If you can't get someone to help you, call animal control, tell them that you have an injured dog and no adult to help you get him treated. They will come out and get your dog and decide what to do.

Perhaps he needs more exercise, play, and training during the day/evening? If he were getting enough, he'd be too tired out to whine all night. A shock collar for a 5 month old puppy would be insane - rehome the dog before you consider doing that.

Some dogs just do this. Clean up the poop right away, before the dog has a chance to eat it.

Just take him outside at regular intervals - don't expect him to let you know that he needs to go out (in the crate or out of it). When he's a bit older, you can try the bell thing again.
